[Update of November 9 at 6:35 p.m.].Faced with the second wave of the coronavirus epidemic, the government supports the latest CNGOF recommendations concerning childbirth and particularly the port of the mask, in a statement of November 9.Thus the Minister of Health, the Minister of Equality between Men and Women and the Secretary of State for Children recall that the "port of the mask in the woman who gives birth is desirable in the presence of caregivers but that'It can in no case be made compulsory ".A position shared by the High Council of Public Health (HCSP) and the National College of French Gynecologists and Obstetricians (CNGOF).
Childbirth and covid: recommendations concerning the port of the mask
Following numerous complaints from women denouncing the obligation to wear the mask during childbirth, the National College of French Gynecologists and Obstetricians (CNGOF) had, in fact, updated its recommendations. Thus, women who do not have a symptom of the COVID-19 can give birth without a mask, even if it remains highly advised. He specifies: "During expulsive efforts, wearing the mask is desirable because it protects caregivers and women itself. It cannot be imposed. We can offer the use of a visor adapted to the face so as to facilitate efforts And communication with the healthcare team. If the patient has no mask or visor, the mask worn by the staff must be an FFP2 mask so as to provide maximum protection to health personnel (+ protective glasses) ". On the other hand, future mothers presented as "possible cases" must imperatively wear a mask. Note that for the time being, maternities are free to impose their own health rules. Olivier Véran, Minister of Health, Élisabeth Moreno, Minister of Equality between Men and Women and Adrien Taquet, Secretary of State in charge of Childhood, also associate with CNGOF to remind that "birth must remain A privileged moment even in this context of epidemic knowing that everyone must be attentive to respecting the safety instructions given by maternity staff ". Ministers also confirm that the mask can in no case be imposed during childbirth.
Presence of fathers to childbirth, even during a cesarean
During containment, dads could only attend childbirth (in certain maternities) and only the 2 hours following the birth of their baby. The device was then relaxed to allow them to support the mother at birth. In a statement published this September 30, the National College of French Gynecologists and Obstetricians (CNGOF) indicates, in fact, that "the presence of the father is desirable to Maximum, including during a possible cesarean (under cover of a charter of visitors, the search for symptoms compatible with a covid, respect for barriers and the effective limitation of travel) ". On the other hand, gynecologists do not advise the systematic search for the clinical signs of the COVI-19 in the birth room, specifying however that this recommendation is to be adapted according to the cases and the local evolution of the epidemic. For its part, the High Authority for Health encourages "to propose screening by RT-PCR at least upstream of the scheduled interventions (cesarean, trigger), and at best before any programmed hospitalization".
Childbirth and covid: Are visits allowed in diapers?
In the last update of his guide on the continuity of monitoring of pregnant women during the lifting of containment, the HAS believes that "the presence of the asymptomatic accompanying in the mother's room can be accepted provided that the Supporter respects the strict rules requested by the establishment and that the establishment has protective equipment and a team in sufficient number to ensure the application of these rules without impacting the proper functioning of maternity ". The CNGOF recommends transforming double rooms into single rooms and, in the event that it is impossible, to ensure compliance with barriers and to prohibit the permanent presence of an accompanying person. On the other hand, facing the pandemic of Covid-19, the durations of stay at the maternity unit were shortened. The HAS specifies: "In agreement with the maternity team, it will be as short as possible: 2 days after the birth of your baby (4 days in the event of cesarean)".
Early release of maternity and risk of newborn jaundice
In a statement published on April 30, the High Authority for Health delivered its recommendations concerning follow -up after childbirth. The HAS specified that an early outing could increase the risk of newborn jaundic (jaundice), which can occur at home."This risk must be the subject of particular attention and an early organization: home surveillance and return to maternity in the event of confirmed jaundice".The HAS also indicated that the realization of the newborn examination 48 hours after the release of maternity was the midwife (the pediatrician being required to examine the child between the 6th and 10th day).
Furthermore, "any newborn woman with COVID-19 suffering must be considered as a carrier of the virus and a diagnostic test is not justified. Precautions are nevertheless to be taken: the isolation of the mother and of the'Child for 14 days is recommended at home. It is recommended not to separate the mother and the child, "recommends the HAS.Finally, the organization of postnatal monitoring must be able to be modulated according to the geographic particularities of the epidemic and access to local resources.
Watch out for home deliveryThe National Union of Obstetrician Gynecologists of France joins the CNGOF to warn the future parents who would be tempted to give birth at home to overcome the absence of the father on the day of childbirth."It is necessary to warn them against the risks to which they would be exposed with regard to the difficulties of the current care system. In the event of a problem, the overwhelmed SAMU may not arrive in time and the reactivity of emergency services for theManagement of the hemorrhages of home issuance will not be the same in the health crisis that we live "warns the union in a press release of March 30. |
